The Sith base on Mustafar, also known as Fortress Vader was an Imperial base and dark side temple built in Mustafar's capital of Fralideja during the Rise of the Empire Era. It was constructed from scratch above a nexus of dark energy by the Sith Empire, and the project lead by Darth Vader with Darth Sidious' permission. The building currently standing is the 9th iteration of the design, created by the ancient Sith Lord Darth Momin through the possession of several various individuals. Following the defeat of the Empire, it was evacuated but remained standing until it was reclaimed by Cinere Autem.
Following Vader's success in killing a rogue Inquisitior, Sidious awarded him with the opportunity to build himself a base on a planet of his choosing, and Vader chose the inhospitable lava wastes of Mustafar in hopes of capitalizing on a nexus of the dark side he had discovered in a cave there. Touching down on the surface with Imperial authorities, the first design he was presented was promptly rejected, and the architect--Colonel Alva Brenne--perished shortly thereafter when an ancient Sith artifact--the Mask of Lord Momin--gifted to Vader by Sidious possessed her companion Lieutenant Roggo. Through Roggo's body, Momin--a seasoned architect--designed a new blueprint for the fortress which capitalized on its position over a Force nexus and presented it to Vader, who killed the possessed Lieutenant and took the mask down into the Sith cave, where he saw a vision about the ancient Sith Lord.
The two collaborated on the creation of the building, though iteration after iteration failed to prove fruitful, with Vader growing more and more impatient as time went by, killing Momin's host bodies as punishment and putting him in others until the ancient Sith Lord was inhabiting the body of a lava flea. It was the 9th design that ultimately was successful, a fortress to serve as a tuning fork in the Force. As Vader got to work attempting to open a gate in the dark side, the two saw it at last working, but he was interrupted by the alert of an attack on the fortress that required his presence. In his absence, Momin opened the gate himself and revived his original body, and when Vader returned after slaying a force of Mustafarians, the two battled, Momin ultimately being killed.

Vanee later had visions of Vader's ghost within the castle, and attempted several rituals to revive the Sith Lord, who claimed that he will rise from the lava wastes much stronger than before. It is unknown what came of the Inspector, and the Fortress was eventually claimed by a cult, who were all slain by Supreme Leader Kylo Ren of the First Order.
The Fortress lay dormant for some time, until in 144 ABY when Sith Champion Cinere Autem sought it out in hopes of reclaiming Mustafar for the then-forming Empire. Heading into the depths of the building with Darth Caelestis, the two were subjected to visions of struggle and traumatic memories, ultimately proving themselves to the dark side and rekindling the nexus that had faltered. Lord Caelestis, perturbed by nearly succumbing to the darker parts of his psyche, left as soon as the mission was complete, though Cinere remained, overseeing the Fortress' rehabilitation and the construction effort. He later found the Mask of Lord Momin in the wreckage of one of Darth Sidious' ships on Ord Mantell and took it to Mustafar, where he tamed the spirit of the Sith Lord.
Facilities
The architecture was clearly Imperial, drawing heavy influences from the style that the Sith had developed over the years: foreboding, sharp, and utilitarian. So utilitarian, in fact, that the temple itself has a lava processing plant in its lower levels, serving as a sort of dam for the rivers. With an exterior made of obsidian, it is resistant to extreme heat and sturdy, with its main points of interest being the two spires sticking out from the main body. They attribute for its height, being 3 times taller than its lower part. Meant to quite literally serve as a tuning fork for the dark side, they were also adapted to serve as towers, featuring various rooms from sparring grounds to ritual chambers. Most notably, they feature the control tower that monitors all activity in the nearby airspace.
The Temple is surrounded by a thick wall and built like a fortress, equipped with a few large landing pads and an inner hangar, as well as a main gate accessible from the mountains it is built on—to the Temple's West, to be exact. The gates are made of obsidian and duracrete and can only be opened from within, operated from a control room. The Temple has hundreds of surveillance cameras. Other than the one in the West wing, there are several other control rooms scattered throughout the building for various other purposes, including (but not limited to), surveillance, fitness tracking (for Sith wanting to track their vitals while training), the medical center, and many others.
The medical center is state of the art and in the lower part of the building. A great deal of effort was put forth to ensure that it would be perfect, and it paid off. It is always active, healing everything from individuals harmed during training to those seriously injured during expeditions through nearby caves. Though most injuries treated are caused by burns from lava and the like, the medical center also stores a few samples of every known antidote available, just in case.
In its hangar are several squadrons of starfighters, ready to defend the Temple from invasion or hostile forces at a moment's notice. When a foreign presence in the airspace is reported, the fighters are quick to intercept it. Within, there are many crews of guards to protect the Temple from everything—whether it be insolent or treasonous Imperials or invading forces. They are soldiers, equipped with lethal weapons and trained to shoot to kill.
The temple has a large section dedicated to administrative duties, an entire quarter featuring personal chambers, a throne room, and several other rooms designated to be used everything from war rooms to bunkers and in between, as well as the Southern Galactic Imperial command center—a state-of-the-art facility that oversees Imperial activity in the Southern part of the Galaxy, from consolidating garrisoned troops to maintaining a smoothly working supply chain.
Below lies a locus for the dark side, a nexus unique in its power, having been used by Vader to seek out a way to revive Padme Amidala, only for him to fail and destroy it before it was rekindled by Cinere Autem and Darth Caelestis.
