Alright, so apparently there are two different versions of the BME Pain Olympics. When I was watching reactions, two people were watching a different movie with the nasties shit.

The one where the guy cuts off his nuts and dick is called "BME PAin Olympics: Final Round" while the "BME Pain Olympics" is a montage of clips of videos of people doing the worst shit to themselves.

One guy cut open his sac and popped out one of his nuts.

I saw 2girls1cup a while back. My friend played it up to be SO disgusting, but it's not all that bad, unless you're really thinking about the shit in your mouth, then it can make you gag. And as for the BME, why is that fun to watch at all? Self mutilation is not nearly as harmless as those two lovely ladies.
